Nursing and health Services         Nurse

School Health Services mission is to maintain, protect and improve the health of all School District #51 students.

The School Health Services Program includes a staff of Registered Nurses and Health Assistants.  The Registered Nurse works directly with school administrators and staff.  A health assistant is assigned to the school and works under the direction of the building principal and the School's Registered Nurse.

School Health services supplement, rather than replace parental responsibility, and are designed to encourage use of the services of physicians, dentists, and community health agencies.  Registered Nurses and Health Assistants are NOT permitted to make medical diagnosis or prescribe medication. 

The following policies/protocols have been developed to facilitate health services for your child:

 .  Each student is required to have a completed Certificate of Immunization 

    at registration. (Click here for 2019-2020 Immunization Information)

 .  A Health History Screening form should be filled out each year and

    returned to the school.

 .  A cumulative health file is maintained for your child.

 .  Medications may be given at school with a permission form filed out,

    signed by the PARENT/GUARDIAN AND PHYSICIAN, and the medication

    sent in a pharmacy-labeled bottle.

 .  The parent/guardian will contacted when a student is injured or ill, basic

    first aid will be given at school.

 .  Please do not send your child to school if he/she is ill with a fever, a

    persistent cough, contagious disease, unidentified rash, or nausea and

    vomiting.

 .  9th grade students will be screened for vision and hearing. You will receive

    written information if the screening indicates need for further evaluation.
